Problem:
Setup the post slug in Toolset post forms.
Solution:
You just need to add the post title field into the post form, and change the label to name, it should be able to setup the post slug too

Solution:
When you add your field using single field block and select your checkboxes field, with the "Field settings" tab you will have option to set what to display when checkboxes field option is checked, you can configure what to display there

The Woocommerce My Account pages are defined in Woocommerce plugin, you can find those files in your website plugin folder:
\plugins\woocommerce\templates\myaccount
