Thank you for sharing the backup file again.
During troubleshooting, I discovered that the form to add a new referral isn't working, because one of it's post reference field "Doctor" has the same field slug ( doctor ) as the one used in the post type slug for "Doctors" post type.
If you'll change this custom field's slug to something different ( e.g. "doctor-field" ), the same form works.
I'm going to share these findings with the concerned team for further review and will keep you updated with the progress through this new ticket.

Just wanted to update that this issue has already reported and a fix is planned to be released in Toolset Form version 2.4.

Hi Waqar, thanks for the update and also the workaround works as you suggested. I will use the workaround, but I have to change all the views where those fields are referenced. That is not a big deal. Do you think Toolset Form 2.4 will be out soon, so I can wait for it otherwise will proceed to make the workaround changes.

I'm afraid, I don't have a time estimate to share at the moment, but I'll notify you, once Toolset Form 2.4 is out.
In general, it is a good idea to keep the custom field slugs different from the post type slugs, so I'll recommend to update such field slugs, anyway.
