I am trying to: add new field post groups, with new fields, associated with newly added post types.

I expected to see: the new post group types; and on the posts types wehre the post groups where added, those fields.
Instead, I got: no post type groups; no post fields in post type.

The problem you mentioned above is abnormal, and I test it in my localhost, there isn't same problem, see screenshot document_fields.JPG
Please check these in your website:
1) Try with another field group name, for example: document_fields 2
2) You have only deactivate plugins, in order to check the compatibility problem, you need to switch to wordpress default theme too, and test again
If the problem still persists, please provide a database dump file (ZIP file) of your website in below private detail box, I need to test and debug it in my localhost, thanks
