I am trying to:
Send notification upon form submission

I expected to see:

Time:
2018-03-22 17:39:30
Receiver:
|email|
Subject:
...
Message:
Hi ...,

Instead, I got:

Time:
2018-03-22 17:39:30
Receiver:
|firstname|
Subject:
...
Message:
Hi ...,

I have resolved this issue by removing the first name field from my sign up form.

- Ideally, I would like to capture this information

Hello,

The problem you mentioned above is abnormal, please check these:
1) You are using the latest stable version of CRED plugin + Other Toolset plugins, for example, you are using Types 2.2.21, but the latest version is 2.2.22, you can download them here:
https://toolset.com/account/downloads/

2) Follow our document to setup the email notification in CRED form:
Automated Email Notifications With CRED
https://toolset.com/documentation/user-guides/automated-email-notifications-with-cred/
CRED User Forms Email Notifications
https://toolset.com/documentation/user-guides/cred-user-forms-email-notifications/

3) In case it is a compatibility problem, please try this:
deactivate other plugins and switch to wordpress default theme, and test again
