CRED Form Created According to Tutorial Not Showing

This is the technical support forum for Toolset - a suite of plugins for developing WordPress sites without writing PHP.

I am trying to:
Create a form for conducting phone interviews to store information in an Applicants database/custom post type.
I followed the steps in the "Adding CRED forms to layouts" tutorial - https://toolset.com/documentation/user-guides/adding-cred-forms-to-layouts/?utm_source=typesplugin&utm_campaign=types&utm_medium=dashboard&utm_term=adding-forms-to-layouts&utm_content=table.
The code shows in the page's edit layout box, but nothing shows on the actual page.

Link to a page where the issue can be seen:

I expected to see:
A form with fields to enter data for my applicants database.

Instead, I got:
A blank page.

Dear Shannon,

The problem URL you mentioned above displays 404 error, so I simply create a new page in your website:

click button "Edit with Layouts", and design it with Layouts plugin:

In the layout add a CRED form cell, please test again, check if it is what you want:
