Hello, I'll be glad to help figure out what is happening here. Can we try these troubleshooting steps?
- Temporarily activate a default theme like Twenty Twenty One, deactivate all 3rd-party plugins except Types, Blocks, and Forms, and temporarily disable all custom code snippets in Toolset > Settings > Custom Code
- Test the Form submission again
- If the problem is resolved, reactivate your theme, custom code, and plugins one by one, testing each time you activate a new component, until the problem returns. Try to determine one source of conflict and we will try to investigate further.
- If the problem is not resolved, I may need to take a closer look.
Okay thanks for the update. Unfortunately, I was able to replicate the same problem in my local test site with the Events Calendar plugin active along with Types, Forms and Blocks. It seems there is a compatibility issue with these active:
- Events Calendar v. 5.9.1
- Types 3.4.14
- Blocks 1.6.1
- Forms 2.6.12
- WP 5.8.1

There is currently an open, known issue impacting post forms, but it seems the problem is more extensive that we understood before, or there may be a separate issue with User forms. At this time, I don't have a great workaround available other than maybe downgrading The Events Calendar temporarily. I need to escalate this to my 2nd tier support team for additional investigation. I'll let you know what I find out after I have some feedback from the team.

If the updates do not show up on your plugin installer page (click the registered link beneath the plugin name to go to the custom installer page) click the the Check for Updates button to update the list.
