Restaurant Review Request Backend.JPG

Tell us what you are trying to do? Attempting to add "Add Media" element to a FORM. Remainder of fields and taxonomies display perfectly and submission to custom post type works fine. Would like to add media (images) from the front-end but that element doesn't display when I view it. I am logged in as an Administrator. Still can't see that Add Media element.
Please note: I am using Divi and I am displaying the form in Builder using the following code (which seems to work fine for all the other elements):
{!{cred_form form='request-for-restaurant-expose'}!}
Page for input form is: hidden link
My block for the (cred) form as it appears backend is attached.
(My first time using FORMS so probably I've done something idiotic.)
Thanks

Hi,

Thank you for contacting us and I'd be happy to assist.

To troubleshoot this, I'll need to see how this form is set up in the admin area and you can share temporary admin login details, in reply to this message.

Note: Your next reply will be private and it is recommended to make a complete backup copy, before sharing the access details.

regards,
Waqar
