I have featured image as a field in the form. All works well when I am posting but when I get back to editing the post and make no changes to the featured, it is lost. That is, the post is saved after I submit edit form but the image is lost. I have to re-open the edit post form and select the image and then post, then it works.
Strangely, when I edit the post for the first time, the image appears in the form (so I think it is uploaded). But when I save the form. The image is lost.
This seems like a bug or an incompatibility issue.
Not sure what could be causing this issue but its definitely not a normal use case as the image should still remain.
In order to ensure that this is not a plugin conflict of some sort I would recommend that you temporarily disable all the non-toolset plugins and check again to see if the issue remains.
Al the plugins I am using are essential for the site and I have a fairly complex setup. Is there another way such as error logs to figure this out. Essentially, I cannot disable the plugins that I am using as they providing essential value.
Ok, no worries. The next thing I would do given the circumstances is to ensure that the issue is replicable on an identical form.
So I would go ahead and create a new form form and place it in the position of the current form then submit that new form to see if the issue remains.
What this does is help us to work through to see where the issue is. If the problem doesn't occur with this new form then we can say that it was an issue with the form you were using.
I have 5 different CPTs and 3 of them have a featured image. This issues is happening on each of the three forms. So I am pretty sure this is true across all CRED forms.
The only other option is to temporarily disable the non-toolset plugins.
However since this will pose some challenges for you I would recommend providing me with a copy of the site so that I can perform the additional tests here locally.
Duplicator plugin has been banned on wprdpress. That's why Waqar used the other one. What else can we do? I can give you access to the site admin details and you can take over from there?
You can run test on:
1) Create a project prompt on Create Project Prompt Page. Add an image.
2) You will be directed to the Project Prompt Post. Then you can click on edit to make changes
3) Edit form will show up with all the details including the image you just uploaded.
4) Save the form and you will go back to the post but your image would be lost. I have a backup image that will show up but not the one you added on the post.
5) Click on edit to edit the post again
6) This time remove the image and add it again to the edit form
7) Save the edit form and this time the post will have the image
Essentially, the image is lost when you save an edit post form without reuploading the image.
I'm still having issues with the site copy as its not loading with any css which makes it extremely difficult to work with.
If possible are you able to setup a copy of the site on your server where we can go through to deactivate the plugins to test?
We essentially need to go through the basic debugging steps before we can go any further as this will guide us as to where to look for the issue.
Also unless these steps are performed I cannot escalate the ticket for further checks as we require that these steps be done and a working copy of the site.
Please let me know how soon you can get a copy setup for us to have a look.
You know Shane, I think Waqar was able to make the copy of the site but it's an issue., I can stop development for sometime and allow you to deactivate plugins on the site for now to check the issues. 🙂

I was able to get the website copy from Waqar and set it up on my test install.
From what I've tested the issue doesn't occur here so it seems that something on your server is causing the images to get removed. See my video below.

Secondly what you can try to do is to enable the media library upload option for the form and see if this helps. This uses a different method to upload the images so we can try this to see if the same issue occurs.
