I can see in the markup on that page that the div containing the form is empty.
It's not possible to see why just from the front end.
Would it be possible to get access to your site to check how this is set up?
Let me mark your next reply as private so that we can get log-in credentials from you—you may want to create a temporary admin user for us to use that you can later delete. And be sure to have a current backup of your site.
I can see that you have given permission in Toolset > Access Control for all users—including guests—to be able to create posts with the form OBM Inserir, so everyone should be able to see the form. (I can see the form when logged in as an administrator.)
It seems like something else is interfering in the display of the form.
Could you please check for conflicts?
Disable all plugins except Toolset, and make sure you are not running any custom code (if you edited the theme's functions.php file, switch theme to twentytwenty, and disable any code snippets added in Toolset > Settings). Then try visiting the page with the form again as a guest to check the visibility of the form. If the problem no longer appears then you should be able to determine the source of any conflict that triggers the problem by a process of elimination.
