Thank you for sharing duplicator.
I checked with the duplicator you shared and I disabled all plugins, still when I login as user "smythe" I can see the issue.
I've escalated this issue to our next level support for further troubleshooting.

Can you please try to make a new form and see it that help you to resolve your issue.
Our Devs found the issue and the issue is when you arranged the meta boxes displayed with the form in different order we see the issue.
I've applied the fix to the database and I can see now it displays the text you added to the form:
=> hidden link
Can you please set the form to use form fields and save it and confirm it works as expected.
Thank you. I can see the problem is no longer occurring on the post you mentioned. It's also gone when I try editing the original form where I discovered the problem. I did not follow your explanation for the cause of this problem. I'd like to understand the cause so I can avoid this in the future. From what I can see the code in the original post form where I first spotted this problem has not changed. Meta boxes are the different sections you see on your edit post form like Form Editor, Settings, Access Control etc..etc..
You do not have to worry about it as the fix will be added for this issue in near future release.
