Editing and saving any form on my site saves the same incorrect data in the form editor. As a result I'm unable to edit any forms.
This happens even if I create a new form. After saving the form the information shown in the form editor (Expert Mode) is nothing like what was there before saving. The same incorrect information shows up in any form which is opened for editing and then saved.
The website is hr.watertowernursing.com but the section of the site where this issue arises is not publicly available. You must login to see the problem.

Yes - I've another user who is talking about the kind of same issue where he is interacting with forms using the author role and he is having the issue.
Can you please share problem URL and access details and share exact steps I should follow to see the issue.

I would also eventually need to request temporary access (WP-Admin and FTP) to your site. Preferably to a test site where the problem has been replicated if possible in order to be of better help and check if some configurations might need to be changed.

I checked the test form you shared: hidden link
I change the post type to "HR Notes" and then again change the post type to "Caregiver Hiring Checklist" and the Toolset form's "Expert Mode" is disabled.
Then I saved the form.
And I can see the same form on the frontend. Am I missing anything here?
The problem occurs when Expert Mode is enabled. Try again with Expert Mode enabled.
When the "Expert mode" is On, we will not change anything within the form's body area even if you change the post that as user will require to handle the edits he made within the form's body area. This is how it works and expected.
If you want to change the post type and want that it should load its associated fields, you should disable the "Export mode" and change the post type.

I'm using Expert Mode because I want to have a unique layout for the form. What I am finding is when I edit the form in the Form Editor (expert mode) and safe the form. The code it shows me in the Form Editor is not what it just saved. It's something completely unrelated. I'm not sure how to explain it differently. I could record a video showing you what happens if you'd like. Or if you could just create a Post Form, edit it in Expert Mode, save it, and look at what's in the Form Editor. It will not be what you expect.

I kept the export mode "On" of the above form and I tied to save the form by adding test "saving text 1", "saving text 2", "saving text 3" etcc.etc..
But i do not see form markup is broken in the backend nor on frontend.

Yes, video will help immensely to locate the issue and I will try to follow the same instruction as per your video. Can you please send me video and once I review the video you shared I will get back to you.

This is really strange.
I loggedin with the user/pass you shared and edit the same "test" form but I do not able to reproduce the issue. Please check the following video:

As you can see with the above video I shared the same steps and do not able to see the problem you mention. Did I missed any step, I dont think so, as I edit the "test" form, "Expert mode" is on and added some text and saved the form and I do not see the things which you can by the way. That is why I shared the video so you can see it.
Maybe you can also check by deactivating all non-Toolset plugins, Do you see any difference? if no:

What if you can share duplicator copy of your site and then I will start deep debugging whats going on with your setup.

In addition to that, we already publish new Toolset plugin version, I suggest to update ALL Toolset plugins.

Yes - now, when I logged-in as the user "smythe" - I can see the issue.
This is really strange. I see you assigned two roles to this user "Administrator" and "Subscriber" - Can you please try to remove subscriber role and check if its working as expected.
I removed the Subscriber Role. It didn't fix things. I added the subscriber role back since it didn't fix things.
As you said - you do not see this issue with your staging site - correct?
- is your staging and live sites using same WordPress version and PHP version?
- What if you duplicate your live site on another server and in order to minimize the cause of the issue, would you mind to follow the basic debug steps:

Could you please try to resolve your issue by deactivating all third-party plugins as well as the default theme to check for any possible conflicts with any of the plugins or themes? - Do you see any difference?
If none of the above steps helps, I will require duplicator copy of your site in order to troubleshoot the issue further. Can you please send me duplicator copy of your site:

I checked the live site and the staging site. They both use the same version of WP and PHP. I will try copying the site and installing it on a different server. I'll post another message when that's completed.
