Hi,
The "create relationship" form doesn't work when a user is logged in.
I've had this issue before but am only getting around now to it.
When you click the drop down to search for the post it to connect to the post in the loop, it finds nothing (and says nothing) even though posts exist.
when NOT logged in it seems to work.

Well - sorry, but I do not see any relationship form with the link you shared. Additionally, could you please tell me which filters I need to use to see the entry and step information that will help me to see your issue.
Regarding JS error, In order to minimize the cause of the issue and to ensure there is no plugin or theme conflict:
Could you please try to resolve your issue by deactivating all third-party plugins as well as the default theme to check for any possible conflicts with any of the plugins or themes?

sorry, that wasn't very clear of me.
I had the form hidden before to guests - sorry about that!
I deactivated plugins and changed theme - still same problem.
It only seems to happen if your logged in.

the form is below and to the left of the "add to cart" buttons
The form is called "add .... to playlist"
when you click on the drop down you should see 2 "playlists" (CPT) there, one called Jazz, and one Rock.
nothing shows up.

Yes - I see the select dropdown with each entry at left side.
could you please share full wp-admin access details so I can check the issue further.

I would also eventually need to request temporary access (WP-Admin and FTP) to your site. Preferably to a test site where the problem has been replicated if possible in order to be of better help and check if some configurations might need to be changed.
I would additionally need your permission to de- and re-activate Plugins and the Theme, and to change configurations on the site. This is also a reason the backup is really important. If you agree to this, please use the form fields I have enabled below to provide temporary access details (wp-admin and FTP).
I have set the next reply to private which means only you and I have access to it.

Well - I do not see any relationship form is displayed with following URL now:

oye - so sorry, I've been doing work on it.
It's in a modal window now - you need to click on "add to playlist" under the cart to get the modal window and see the relationships form.

Well - I checked and clicked on the "add to playlist" - I see there is one dropdown (select2) available and its loading the options just fine.
Could you please share details what exact function you want to make it work or do you want to add any relationship form?
Please confirm that you logged in to the site as I said above.
It seems only to be a problem for logged in user.
It always works just fine the first couple of times. but the more you move around the site it ALWAYS eventually stops working.
If you just went there once and tested, then yes it probably worked.

Yes - it's strange, now I can see the issue and yes, while logged in.
Checking further. I will get in touch with you as soon as I know more.
I've tested the issue with minimum setup (Only Toolset plugins active + Default theme) and I do not see the issue.
Checking now by installing the Divi theme. Will update you here as soon as possible.
Well - I checked Divi with same as your structure that Divi builders one row should display search form and another row should display the search result.

Now, I just dont able to reproduce the issue. I've also added the Toolset Form to create the entry at the top and just below it the relationship form. Please check this video as evidence:
