Hi, please try the following troubleshooting steps:
- Temporarily deactivate all plugins except Types, Views and Forms and activate a default theme like Twenty Nineteen.
- Submit the Form once again and check the notifications that were sent. If the problem was resolved, reactivate the theme and plugins one by one until the problem returns.
- If the problem was not resolved, please edit this Form and open up all the notifications. Take screenshots showing all the notifications, their triggers, and their recipients. Please attach those screenshots for me to review and we can go from there.
