Problem:
How to "merge" the information of many Custom Fields Fields (City, street, state, etc) to the new Toolset Maps Address Custom Field
Solution:
The current Toolset Maps plugin uses a single Field (Types Address) to store the Address.
If you have a existing database with the older setup (different Fields for street, city, etc), you can enable the Toolset Maps Legacy Mode under Views > Settings > Toolset Maps.
But recommended would be to use the new Toolset Types Address Field from scratch.
There is a possible Custom Code you can create yourself or by Certified partners, to update old Fields to the new Address Field:

Toolset does not support this Custom Code and therefore it requires advanced PHP Knowledge.
This is the technical support forum for Toolset - a suite of plugins for developing WordPress sites without writing PHP.
