I am trying to: add a CRED form shortcode

Link to a page where the issue can be seen: hidden link

I expected to see: CRED form with footer but instead the shortcode breaks the page

Instead, I got: server 500 error. i contacted server admin and they gave me this response:
[07-Nov-2017 18:58:08 UTC] PHP Fatal error: require_once(): Failed opening required 'field/class.radio.php' (include_path='.:/opt/cpanel/ea-php56/root/usr/share/pear') in /home/downtoncareersco/public_html/wp-content/plugins/cred-frontend-editor/application/controllers/form_rendering.php on line 83
