if i add a CRED form shortcode to a page it generates a server 500 error.

I am trying to: add a CRED form shortcode

I expected to see: CRED form with footer but instead the shortcode breaks the page

Instead, I got: server 500 error. i contacted server admin and they gave me this response:

Thanks for the feedback, we've published an errata for this issue: https://toolset.com/errata/exception-fatal-error-failed-to-open-fieldclass-radio-php-shown-on-page-with-cred-form-shortcode/

Please follow the steps mentioned in this Errata and let us know if it fixes the issue.
