Couple of days ago I created Post Form which worked.
From Yesterday form has broken design.
In Toolset editor everything looks fine and also similar forms works fine.
Before I starts to redefine form I need to know what causes this to avoid future problems.
Is there any documentation that you are following?

I have tried the URL you mentioned above, it works fine, I can see the post form correctly, see my screenshot forms1.jpg.
Is this issue resolved? there might be some cache issue in your website, if you are using some other cache plugins in your website, please try to clear the cache and test again.

In between I add to page new form for same post. So page contains 2 cred forms.
Upper one is newly created and bottom one is issued form.
On bottom of your screenshot is start of broken form.
Check bottom form.
