I am trying to upload Featured Image from Cred form

I've performed some tests on my website with a similar form and settings, but couldn't reproduce this issue.
Was your website migrated from a different domain/address than the current one ( hidden link )?
I'm asking because we've recently identified an issue which affects only those images which were uploaded when the website had a different domain/address than the current one. In that case, such images are not set as featured image, when selected through Toolset Form.

As a simple test, you can first try to select a featured image which was uploaded before the migration and next use an image which was uploaded after the migration.
Note: In my test with your form, the newly uploaded image is selected as a featured image, correctly.

If it is confirmed through your tests too that this is only happening with the pre-migration images, this means it is the same issue and a fix for this has been planned to be included in Toolset Forms version 2.5.1.

Yes the settings should be all fine so there is a bug somewhere.
I have indeed migrated the site BUT that is a week ago, and this problems started just two days ago.
Please feel free to replicate the issue on my site.

But the image IMG_2521.jpg uploaded just before does not work: It seems very random what images work and not work
The filenames shoud be good.
I see from your testing on the site that you also have some images that are not stored. But it worked at your third try , that is the same that I experience, some works and some dont.

- In my first two submissions, I selected "IMG_2521.jpg" image, the same that you reported.
On the third attempt, I used a newly uploaded image and it worked, as expected.
This and your observation about the "wmotors_muse_banner_img_01.jpg" image suggests that this is the same issue, which affects the images uploaded before the website's base address change.
However, for absolute confirmation, it would help if you could share the website's clone/snapshot so that I can investigate this from my own server. Since this troubleshooting requires checking database entries, I'm afraid, WordPress admin access alone won't be enough.
