This thread is resolved. Here is a description of the problem and solution.
Problem:
The issue here is that whenever gravity forms is active our Toolset Forms Icons on the edit page no longer display.
Solution:
This is a known issue by our Development team and an errata page has been created for this.

I am trying to: delete custom field in Post Forms
Instead, I got: cannot delete custom field (missing trash bin icon)
It occurs only with Gravity Forms plugin active (ver. 2.4.23, and 2.5-rc-1)
GF No Conflict Mode activated
