I am trying to: Create a new CRED User Form to allow site visitors to register for a site and become a Member based on custom User Roles that I created in Access Control.

I expected to see: The CRED User Form be created.
Instead, I got: An error message that reads: "Could not create the CRED User form"

Please check these:
1) You are using old version of CRED plugin + CRED commerce plugin, please upgrade all Toolset plugins to the latest stable version, you can download them here:
https://toolset.com/account/downloads/
2) Deactivate other plugins and switch to wordpress default theme, and test again
3) If the problem still persists, please provide a database dump file (ZIP file) of your website in below private detail box, also point out the problem page URL, I need to test and debug it in my localhost, thanks
Thanks for the details, I can duplicate same problem in my localhost, and the problem is very strange, it only occurs to the custom user role " Professional", I have tried to create another custom user role " Professional 2" with same settings, and it works fine. see screenshot: Professional-2.JPG
I will escalate this issue to our 2nd tier supporters, currently you can try these in your website:
Edit the layout, add a "CRED user form" cell, choose option " Use an existing Form", and select the existing form "Create User - Professional", and test again
