Problem: I would like to create an image gallery that alternates with a single landscape image on one row and two portrait images on another row.
Solution: Use a Toolset Gallery block with a custom Collage display mode, or a more complex system of Views and galleries, depending on how the custom fields are structured.
Q1) I have tried to figure out how to create a bootstrap grid but have not been successful.
Q2) I will have to go to the country page and manually tick the new city / region taxonomy every time a new city / region is added. Not a huge problem but great of there is a way to do it dynamically.

Q1) Since you are using Bootstrap framework in your website, so you can format the outputs using Bootstrap, for example:

Use Views plugin to query posts, with a taxonomy filter, the only problem with this approach is an issue I thought I had solved a long time ago but apparently wishful thinking and that problem is that this solution is also not taking the hierarchy into consideration.
Solution:
Views plugin is using WordPress class WP_Query to query posts, by default, the taxonomy filter will include child terms for hierarchical taxonomies.
In your case, you can try with Views filter hook "wpv_filter_query" to change the parameter "include_children" to false, for example this thread:

How to only output fields (and any labels) if the field has a value, specifically in Elementor templates.
Solution:
You wrap the relevant section inside wpv-conditional shortcodes, as described in the linked documentation, so that you have something like this:

When inserting in an Elementor template you need to make sure this is inserted in a module which parses shortcodes, such as the Text or Shortcode modules, but NOT the HTML module, which does not parse shortcodes.
Problem: Why would I use Elementor to build a page instead of Toolset?
