Is that a new test page you created or the same one.
I don't understand the set up of the existing test via submitting a form, as I stated in my last reply.
I have passed this issue on to the developers, who should be able to fix the problem with the new short video url format for the Youtube block fairly soon, and that should fix your issue however you have set it up.
So I will mark this thread as escalated, and will update you again when the fix is available.
I'm just cleaning up old tickets and note that if you are using the latest Toolset plugin versions you should no longer be experiencing this issue, so suggest closing here.
