I checked the debug information you shared and I see that you are using the latest version of Toolset plugins.
However, I would like to have more details. When you say "Admin does super slow when its active.":
- Do you mean that the whole admin section is slow when Forms plugin is active or is there any specific pages that is slow?
I just tried to reproduce the issue by installing the latest Types, blocks and access plugin which I can see with the debug information you shared and I do not able to reproduce the issue.
Can you please share admin access details so that I will try to troubleshoot the issue further.

I would also eventually need to request temporary access (WP-Admin and FTP) to your site. Preferably to a test site where the problem has been replicated if possible in order to be of better help and check if some configurations might need to be changed.
I have set the next reply to private which means only you and I have access to it.
Once you log into cpanel, scroll down to My Application and you will see WordPress "Bill Reminder" Click on this. Once you are in application area, you can click on the admin link and it will automatically log you into the admin.
If you must have direct access, I will need to reset the password.

I've activated the Toolset Forms plugin on your install and I checked the following pages:

I do not see any significant issues. All of the above pages are loaded between 6-8 seconds on your server.
Then I installed the duplicator plugin on your site and downloaded the duplicator copy on your install and installed in on my local test server and I can see it takes 5-6 seconds. Even I installed the "Query Monitor" plugin to check and verify if there is any slow queries but I do not find much that is should affect the admin slow time.

I suggest you should try with a different server - do you see any difference?
Thanks for your assistance. It turns out to be our corporate UTM firewall.. It mustn't like something in the code when it scans it. When I by pass by using my Mobile 4G connection it works perfectly.
