I have created a user form and added a notification template.
The notification does not get sent when the form is submitted.
Test emails do go through.
I can give you access, if you will please enable private fields.

I forgot to update the user after generating the new password. It is working now and I have updated the private fields above with the working password. My apologies.

No idea what I was doing wrong or missing, but I swear it wasn't sending, and I tried multiple times.
