I have a User Form with a couple of custom fields. When I view the form I get the errors:
Your Profile Image
There is a problem with your-profile-image field. Please check CRED form.

Profile Highlight Colour
There is a problem with profile-highlight-colour field. Please check CRED form.

I've disabled all plugins other than Toolset, changed the theme to Twenty Seventeen, and verified that all Toolset plugins are the latest versions.
I have also created a brand new test form and an new custom field with the same results.

Here's a video of my settings:

Okay I don't see any open issues that indicate the same problem, and a quick test on my local didn't have the same results. A few questions:
- In your User Profile, is the custom image field appearing as expected? Is an image actually saved in the image field, or is it empty? If it's empty, can you add an image and see if the results are different on the front-end, and vice versa?
- Twirl open the image field element in the Form builder. What is selected here?
- Are any custom scripts active in Toolset > Settings > Custom Code?
- Are any custom JavaScript scripts applied to this Form?

In the standard user profile page I could see the two fields but it wasn't an optimal experience 🙂

When logged in as an administrator I was able to see the files in the media library but the add button wasn't functional. I pasted the URL for an image in the image field and was able to save the profile.

Once that was done, my front end Toolset Form started working for me as an administrator and as my regular user test account 🙂

Might it have something to do with the fields being created *after* the user profile?

I don't think so, that's a normal workflow.

I see the "sorry you are not allowed to upload files" message in the Media Library in your video...that's not normal. Is that happening if you deactivate Toolset and try to upload an image to the Media Library directly when logged in as Admin?

I can't reproduce the issue on the site I'm working on so it was just the first time. Once I added content from the back end things seemed to have been fixed up.
I'm slammed with work right now and just don't have the time to create another blank test site 🙁

BTW, whoever added the ability to use the media library or not should get a gold star 🙂
