==> I suggest to reinstall manually once to rule out the installer issues.

==> Well - first we would like to see ALL Toolset plugins should be updated on your install, once done, you need to follow the basic debug steps to check if there is any conflict between any third party plugin or theme you are using.

Could you please try to resolve your issue by deactivating all third-party plugins as well as the default theme to check for any possible conflicts with any of the plugins or themes?

To follow the basic debug steps, please setup a test site where you should run some tests.
