I am trying to:
Save a form created with Toolset that uploads images and text to create a gallery page. It has been working for two years and now when the user clicks the “save” button they get a 400 error.

This cannot be seen on the website unless you are logged in. And only if you are logged in as an editor and using the form template.

Can you give as much information as you can?
Were there any updates, changes in the setup, changes in the hosting service provider? Something that we can work with here?
A 400 error on Toolset Forms save is usually caused by the submit request being blocked (often by cache/security) or by the AJAX submit mode.
The steps I suggest with the limited information that I have at hand:
- Quick test: turn OFF AJAX submit
Edit the Form → Settings → uncheck “Submit this form without reloading the page (use AJAX)” → save → test again.
- If the form uploads images, switch to the Media Library uploader
Edit the Form → Settings → enable “Use the WordPress Media Library manager for image, video, audio, or file fields” → save → test again.
