This thread is resolved. Here is a description of the problem and solution.
Problem:
The issue here is that the user is unable to add a new relationship form.
Solution:
In this case the user didn't have any post relationship that required the use of a Relationship Form.
To get the relationship form you need to create a many to many relationship

I have already created relationships between different post types and I expected to see the relationship form in the backend but I did not see it. Furthermore when I tried to add a new relationship form, the create new button is disabled.
I tried to replicate the problem on another site in the same network but for some reason everything is working on the other site.
I've already tried to clear the cache in my browser as well as use different browsers. I've also tried to deactivate all plugins which are not required.
I've tried to create a one to one, one to many and many to many relationship type, and for my test site, all of them are working, and the boxes for the relationship show up in the backend. However for the mainsite the boxes in the backend is simply not showing up even after I've checked SCreen Options to make sure that everything is showing.
