The issue was - you are using "Content control" plugin that was somehow overriding the permissions.

I've disabled the "Content Control" plugin for a moment and then re-saved the forms permission from Access Control: hidden link

And resaved the page where you added the form:
- hidden link

And then activated the "Content Control" plugin and I can see the form on frontend.
