Hello. Thank you for contacting the Toolset support.

It seems you are missing the form wizard.

Can you please navigate to:
=> Toolset => Settings => Forms tab => Forms Wizard and checkmark the setting "Create new forms using the Forms Wizard"

I hope that helps.

First of all - After checking to debug the information you shared with us, I found that you are using few outdated Toolset plugins. We always recommend running your site with the latest stable release plugin version.

*** Please make a FULL BACKUP of your database and website.***
Could you please update ALL Toolset plugins to it's latest officially released version. You can download the latest plugin release from your accounts page:
=> https://toolset.com/account/downloads/

In addition to that, please update WordPress to latest version.
=> https://wordpress.org/download/

Even after update you still see the issue, please check for plugin conflict where you should disable the plugins one by one to check what plugin is causing the issue.
