I am trying to:

Link to a page where the issue can be seen: Brow Beat Studio, Dallas most Advance Microblading Experts studio

I expected to see: a working form

Instead, I got: Send us an email:

[contact-form to='derma_aesthetic@yahoo.com' subject='Email for Derma-Aesthetic web site'][contact-field label='Name' type='name' required='1'/][contact-field label='Email' type='email' required='1'/][contact-field label='Message' type='textarea' required='1'/][/contact-form]

Those ShortCodes are not from Toolset.
I also do not see that you edited that Page with Toolset at all.

Does the issue disappear when you disable Toolset?
If not, then the problem is not related to it.

I suspect the Plugin that produces those ShortCodes is either disabled on your site or has a BUG.

Please try to find the Plugin that produces those ShortCodes and make sure it is active.

If then the issue remains, disable Toolset (perform a backup before you do this) and check again.

I also see you tagged this Ticket with "Toolset Starter", but I do not see that Theme on your Website.

If then the problem remains, let me know, but I suspect this is not related to Toolset.
