This thread is resolved. Here is a description of the problem and solution.
Problem:
When adding a Field and Text block, selecting Add Field or View, and inserting the Forgot Password Form, entering the reset password page URL does nothing.
Solution:
Ensure Toolset plugins are updated by navigating to WordPress Dashboard > Plugins > Add New > Commercial (tab) and clicking Check for Updates. After updating, follow the guide to set up the lost password feature:
