[Resolved] How to create front end entry form as per the screenshot image

I am trying to: create a front end user entry form as per displayed in the image attached / uploaded

Instead, I got: -- I am totally new to types and toolset and cred etc...

Also please help me how to structure CPT to achieve the form as per image

1. create a front end user entry form as per displayed in the image attached / uploaded

CRED creates a particular kind of output: it uses Bootstrap HTML, CSS and JS.
That means if you autogenerate the Form it will be a full Bootstrap form.

2. To customize it, you can use HTML and CSS.
We do not provide custom HTML or CSS as this is a required knowledge to use Toolset.
We do have a DOC on how to address particular sections of CRED:
https://toolset.com/documentation/user-guides/styling-cred-forms/

To acheive your goal, you need to customize your Form using Custom HTML.

3. Also please help me how to structure CPT to achieve the form as per image

The structure of your Post Types is mainly managed with Types.
It is also a different Support Request, which I have to ask to report in another Ticket:

The Form you show in your screenshot will require elaborated Custom HTML and CSS.
I reccomend, for ease, to use Bootstrap as it's natively delivered with.
