Currently i'm waiting on our 2nd tier to have another look at the issue for you. I've also removed the log from the thread as well as provided it to our 2nd tier team as well.

Can you make a staging/testing site available on your server?

I'm unable to reproduce the problem with the copy of the site shared by Shane after replacing the Forms plugin, and I'd like to be able to do some debugging on a site that currently displays the problem that is not your production site.

Let me set another private reply to get access credentials.
