Using the pro Kadence theme and blocks, I have set up a sticky header with a styled site name and main menu. Using Toolset, I have created a content template for one of my custom post types (Individuals). When viewing a test post on the frontend, I see the following problems:
1. My header with styled site name and navigation are missing. Instead, the site name and post title (which I do not want to show) are in the upper left-hand corner along with an unwanted black centered rule.
2. Footer is missing.

Settings on the Content Template tab under "Theme options (Kadence)"
1. Scroll to Top: Use Theme Settings
2. Sticky Header: Use Theme Settings
3. Footer: Use the current theme's setting

Settings under the Customizer > Custom Post Types > Individuals Layout
1. Show Title? set to no
2. Title Elements: Title visibility set to no
3. All other settings duplicate what I have set up under Page Layout and Blog Posts > Single Post Layout in the Customizer with Show Title set to "No."

What am I missing?
