I expected to see: The correct form slug and content
Instead, I got: My form defaults to a published page's slug and content
To elaborate on my issue:
I have attempted to fix this issue for the past 2 days. I have deactivated all plugins (except Toolset Types and Forms) and even changed my theme to the wordpress default theme Twenty Twenty. The issue still remains: I can not edit and save ANY of my forms because for some strange reason when I open any given form , the slug defaults to "cart" and the form content is the woocommerce shortcode "[woocommerce_cart]". This prompted me to move my pages to "trash" status one by one. Each time I deleted a page, my forms simply updated to another page's slug and content. After deleting ALL OF MY PAGES, my forms finally displayed the correct slug and content.
If this makes a difference, the developer console is full of "non-unique id" error messages. The first set is error messages about custom fields I created via Toolset, and the rest are about CRED fields. If I remove my custom fields from their custom field group (so they don't show in the form scaffold), the error messages persist but only for CRED fields.
I truly hope I explained this issue with clarity. Any help would be greatly appreciated!
We had a report of something similar just recently, I'm wondering if this is the same issue.
In that case the cause was that on the page where you edit the forms, it is possible to drag and drop the various metaboxes to re-order them, and the plugin code wasn't prepared to handle that. We've identified that problem, which should be fixed in the next main Forms release.
I don't know if your issue is the same, but the symptoms are similar.
I've shared a screenshot of the normal default order (I collapsed the sections to fit them on the screen).
Have you changed that order in your forms? If so, I think it's the same issue, but if not then it must be something else, and we'll likely need a copy of your site for testing.

As Nigel shared, this is known issue to us and there is a patch available.
In order to apply the patch, I will require the same login access details using which you are currently loggedin and see the issue.
*** Please make a FULL BACKUP of your database and website.***
I would also eventually need to request temporary access (WP-Admin and FTP) to your site. Preferably to a test site where the problem has been replicated if possible in order to be of better help and check if some configurations might need to be changed.

I'm not sure if I should start a new thread or not, but I just wanted to inform you all that I am having the same issue with my forms. Unfortunately, the fix you applied is no longer working.
