I took a look at this for you, it seems that this could be an issue on your end as I was not able to replicate and I also have a customer with this exact plugin but the issue was not replicable so I suspect a server issue on your end.
Could you delete the 2 instances of the Access plugin and download a fresh copy of the access plugin from our site below? https://toolset.com/account/downloads/
Also could you try downgrading your php version from 7.4.6 to version 7.3 and let me know if the issue still remains.
