"Sorry, you are not allowed to access this page" message appears after network-activating Toolset Access

Symptoms

After upgrading or activating Toolset Access in a Multisite installation of WordPress, (Network Admin -> Plugins page), the following warning appears:

Sorry, you are not allowed to access this page

Additionally, if you use WordPress Debug feature you will see the following PHP Warning:

Notice: Use of undefined constant AUTH_COOKIE - assumed 'AUTH_COOKIE' in .../wp-includes/pluggable.php on line 776
