A form to publish posts has settings for “Include the Add Media button in rich text fields” and “Use the WordPress Media Library manager for image, video, audio, or file fields”, both of which are broken in the latest version of Forms (2.6.10).
In each case the settings behave as if checked, even when not.
The developers are aware of the issue and will work on a solution.

The fix for this has just been published.
If you don’t see an update notice on your plugins page, click the registered link for one of the Toolset plugins to take you to the custom Toolset installer page, and use the Check for Updates button, otherwise you can download the latest versions from your downloads page at toolset.com/account/downloads.
