When adding a Toolset Maps address field to a CRED form, it is not displayed on the front-end. Instead, an error message is shown. Depending on the server configuration, a PHP error might appear on the page or in the server logs.
This issue will be solved in an upcoming hotfix release for CRED.
In the meantime, this patch can be used as a workaround against Toolset CRED version 1.9.1. To apply it, unzip it and upload the file it contains to replace the one with the same name, located in the ../application/controllers/field_translation/command/ folder.

This has fixed the error but it no longer shows the drop-down when entering an address so you can select it and move the pin on the map.

I think you found a different problem here.
While trying to replicate it, I notice that in some cases, when the same page has more than one address field to edit (when creating a post with several address fields, either in the backend, or with a CRED form in the frontend), only the first one was properly initialized, and the rest of them displayed a message “We could not connect to the Google Maps autocomplete service.”.
After some debug, I think this is related to the way Google Maps is answering to our initial check that their API is reachable, when asked politely more than once. Maybe they changed how they answer to such multiple and simultaneous petitions, I am not entirely sure, but we are already working on a solution. We will have an errata about this specific issue published later today.
In any case, it would help us a lot of you could open a specific support ticket with all the information and steps to reproduce your issue.
