Core Media and Text block errors when Toolset Forms active

Symptoms

With Toolset Forms active, since the WordPress 6.1.1 update, if you insert a Media and Text block into a page or post it generates several JavaScript errors, visible in the console.

Likewise, if you have an existing page with a Media and Text block and try to replace the existing media, it triggers the same errors.

There is a separate issue, which is that when using Dynamic Sources for the image, the block does not display correctly on the front end.
