Taxonomy Error Notes.jpg

I am trying to: Setup a page that has nested taxonomies.

I have configured a basic hierarchical "focus" taxonomy.
I created a new "focus" taxonomy entry entitled Foundation
I created a second "focus" taxonomy called "cautions" and used Foundation as the parent.

What I got was A Foundation Taxonomy AND a Foundation>Caution Taxonomy. So I deselect the Foundation because it's already established by "caution".

I created a second post and added the focus labeled "history" as a subset to the Foundation parent focus.
What iI got was History by itself, Foundation with two options under it to select from) then Foundation with the same two options under it to select. That makes no sense.

So I created another post to ad the "Manifesto" subfocus under the Foundation focus and got several copies of the foundation hierarchy to choose from (all the same) and Manifesto appears on its own. Not under Foundation.

What's interesting is that after creating the hierachy with manifesto in it, it doesn't show up under Foundation. But the next Post I create it will be there.

Link to a page where the issue can be seen: This is working with the default editor creating a custom post type (with Toolset)

I expected to see:

After adding the Sub Focus type, I expected one header Foundation with several Sub Focus listings under it.

Instead, I got:

Multiple copies of incomplete hierarchies to choose from.
