[Résolu] cannot add child posts to parent

i build a form for a cpt users
in preview it looks perfect
than i added in this users cpt a template with a create childs post link like this:

[wpv-post-title]
[types field='tehudat-zehut-family-member' format='FIELD_NAME: FIELD_VALUE'][/types]
[cred_child_link_form form='11452' parent_id='-1' text='Create new' target='_self']

[cred_form form="create-new-family-members"]

i created a page with this form and when i go to the page i see:

add new group member
[cred_field field='@yeled-ora.parent' select_text='--- not set ---' class='form-control' output='bootstrap']
[cred_field field='form_messages' value='' class='alert alert-warning']
חבר קבוצה Name [cred_field field='post_title' post='family-member' value='' urlparam='' class='form-control' output='bootstrap']

in place of the form...
what do i did wrong?

There was a page template in the add child page wrong defined
