A Membership Form Using Cred Form with Woocommerce sends to a 404 Page

I am trying to: Submit a Membership form that is using Cred Form with Woocommerce so people can pay for membership

Link to a page where the issue can be seen: lien caché

I expected to see: The Woocommerce Checkout page after submitting the information. Has worked for years and stopped all of a sudden

Instead, I got: A 404 page not found after sign-up
