This is the first time I've seen this happen but recently had two users upload on the site. I believe they uploaded the same filenames. But they are on different posts and were different images, but WP and Toolset saved them with the same name so they didn't work properly. Is there anything to keep this from happening?

It's possible (still digging) that actually one was uploaded as a png and one as a jpg and our image optimizer tried to convert the png to a jpg. So I guess is there any setting that will force it to give it a unique name (regardless of extension)

Hi there,

Toolset has no say on the image naming when it comes to saving the image to WordPress Media Library. That is something WordPress does.

I did a quick search and found the plugins below that might help you, but you need to test.

https://wordpress.org/plugins/phoenix-media-rename/

https://wordpress.org/plugins/filebird/

Thanks.
