It should be a CSS issue, but I can not see the problem in the URL you provided above, it seems to requires a logged-in user, please provide your website credentials in below private message, I need to test and debug it in a live website.

It is a blank page, I am not sure where I can see the problem.

So I have done some test in your website:
1) Edit the content template "Template for Pros 1",
2) Add a Toolset Forms block, insert form "form-for-pros"

3) Test it in fronend:

Fill the form and submit, it works fine, see result here:

The page should not be blank, it has been published, that is the problem it blank.
