There is no easy way to achieve this and this for sure need custom programming. This is likely to be related to your web server's configuration files, such as ".htaccess" for Apache web server.

There is rewrite option available with CPT:
=> Edit your post type => "Options" section => Rewrite option

Where you can set your custom URL format but the custom URL structure you desired will require custom work.
