Getting an error when i click on create form

This support ticket is created 3 years, 1 month ago. There's a good chance that you are reading advice that it now obsolete.

This is the technical support forum for Toolset - a suite of plugins for developing WordPress sites without writing PHP.

Everyone can read this forum, but only Toolset clients can post in it. Toolset support works 6 days per week, 19 hours per day.

Clicking create new form generates the attached error.

I have tried turning off my theme and all plugins other than toolset and i have the same issue.

This is not a live site so feel free to change anything needed.

site is at warehouseframes.agamastudios.com and the

admin is at warehouseframes.agamastudios.com/wp-admin
user- toolset
pass - removed
