We are using Toolset types plugin. We created a custom post type called Grade Guide. I am wondering if we can remove "grade-guide" from the individual page URL.

There is no native way to remove the post-type slug from the single post permalinks.
In order to achieve this - You'll have to rely on some custom rewrite rule workarounds, as discussed in the following resource:
