Yes, It sounds like JavaScript error. Can you please check is there any JavaScript error inside browser console. If you use google chrome just press CTRL + SHFIT + I to open Developer tools and JavaScript console should be visible.

I've just checked your website and everything works fine for me. There is no any JavaScript error, and I can generate shortcode without problems.

But, there is one thing I noticed in your video, you are trying to enter page title - "TitlePage" that actually doesn't exist (under "Child form page" section). Please have in mind, in "Page" field you have to enter title of existing page, when you type first 3 letters plugin will show you list of suggested pages in drop down menu (please see screenshot)
