Hello and thank you for contacting us.
Please check if you have the latest versions of Toolset plugins.

Can you test with all non essential plugins disabled and with a default theme like Twenty Fourteen?

Can you enable debugging in WordPress and provide us the log?
You can do this by switching define('WP_DEBUG', false); to true and using this code inside wp-setings.php:

// Tell WordPress to log everything to /wp-content/debug.log
define('WP_DEBUG_LOG', true);

// Turn off the display of error messages on your site
define('WP_DEBUG_DISPLAY', false);

// For good measure, you can also add the follow code, which will hide errors from being displayed on-screen
@ini_set('display_errors', 0);

Well I did that,
I actually tore down the site,
installed wp again clean,
Uploaded and installed / activated Types / Views / Module Manager,

Added your code to wp-config as opposed to settings as you mentioned.

It creates a debug file but nothing in it and still end up on the 404

I managed to figure out the path to the module in the 404 url and installed the module manually.

Do you still need assistance with this?
